When you submit the consultation request form, we collect the information you provide — your name, email address, phone number, the procedure you're interested in, your preferred location, and any goals or questions you choose to share.
How we use it
We use your information solely to respond to your inquiry and to schedule and follow up about a consultation with Dr. Sean Satey's practice. We do not sell your information.
How it's handled
Form submissions are processed through Cloudflare (including Cloudflare Turnstile for spam protection) and delivered by email to the practice. A record of your submission may be retained to manage your inquiry.
Sharing
We share your information only with the practice and the service providers that help us deliver and store your message. We do not sell or rent personal information to third parties.
Your choices
To request access to, correction of, or deletion of the information you submitted, contact the practice at (424) 500-3320.
